The functions of MGH Service coordination or case management include but not limited to; securing, developing, implementing, coordinating and monitoring of a comprehensive therapeutic programs provided to the individual as identified in the Individual Service Plan (ISP). MGH Service Coordinators are responsible for securing and coordinating appropriate services for consumers. An Organizational central point or Service through all services are directed and coordinated to attain the valued outcome.
The Agency’s Service Coordination is a transitional road map for the individual’s to attain their desired outcomes at any given time.
Mary Golden House works with all service providers to make all transition or waiver services possible by doing the spade walks for you and your loves ones
The Agency Service Coordinators will be responsible for providing consumers with all the information for some resources vital for the individual living and integration into the community.
Mary Golden House specialists have the valuable information and knowledge for services that will serve individuals with reasonable outcomes and productive happy future.
The service coordinators treat each case personally and in accordance with the family or individual’s valued outcome.
All services provided by Mary Golden House staff come from the heart and are provided as the service coordinator will do it for him or her family or self. On regular basis as needed, MGH service coordinators involve in individuals’:
Home Evaluation as a safe guard; Counseling or Rehabilitation if needed; Financial Planning or money management if individual works or receives SSI etc; Individual Service Plan re-evaluation with other service providers. Assess the Life Skills Aide Report or Safeguard plans; School progress report if individual is in school; and job training or vocational activities.  MGH Service Coordinators initiate and oversee the assessment and reassessment of the individual’s level of care.
Mary Golden House offers various assistance to the families of the consumers by assisting them with applications for guidance ship, legal or social correspondences, referrals, social and emotional counseling.
